Forms at Trainer Level
This kit contains the following Forms.
1. The
SIYB Entry Form
This Form is used as Application form (it replaces GYBI/SYB
and IYB Application Forms) and can also be used as Training
Needs Analysis Tool, as it identifies areas in which trainee
may need training. The Form is also the base-line for Impact
Assessments. It is used before or at the time of the TO(P)E.
Trainers are asked to keep copies in a file for assessment
purposes after training.
2. The
SIYB Selection Tree
This Form is for the Trainer to use when assessing the SIYB
applicants. Based on the information on the SIYB Entry Form,
the Selection Tree will guide the Trainer on the appropriate
SIYB training intervention to be recommended for the participant.
3. The
SIYB Assessment Form
This form is used at least 3 months after the SIYB TO(P)E.
It assesses impact of the training and replaces the SIYB Evaluation
Questionnaire and the Business Performance Card. It can be
used by Trainers, Master Trainers and by RPO.
4. The
SIYB Activity Report
This very important tool summarizes information about the
TO(P)E conducted and provides basic profiles of the participants.
Trainers are strongly encouraged to send a copy to the Master
Trainer who can then send it by email or fax to RPO. All TO(P)Es
that remain unreported, are not entered into RPO’s data-bases
and therefore not taken into account when assessing impact
etc.
5. The
End of TO(P)E Seminar Evaluation Form
Filled in by participants at the end of the TO(P)E to provide
feedback to the trainer about the level of appreciation of
the seminar.
6.
The End of Seminar Worksheet
Used by trainers to calculate final score of appreciation
of a seminar.
7. The
Daily Reaction Evaluation Form
Forms at Master Trainer Level
This kit contains the following Forms.
1. The
SIYB Training of Trainers Application Form
This an application form used to access the potential SIYB
trainer before the TOT. It can also be used a training Needs
Analysis Tool, as it identifies areas in which the potential
trainer may need improvements. The Form is also the base-line
for selection of potential trainers for interviews. It is
used before the TOT. Master Trainers are asked to keep copies
in a file for assessment purposes after training.
2. The
SIYB Interview Guide
This Form is for the Master Trainer to use when assessing
the potential SIYB TOT applicants. Based on the scoring, the
Master Trainer will select the applicant for the TOT seminar.
3. The
SIYB Trainer’s Performance Card
This card is used after the TOT. It helps the Master Trainer
keep truck of the Trainers activities in the field. It is
also used when assessing the Trainers for certification and
general performance. This tool can also be used by RPO.
4. The
SIYB Activity Report
This very important tool summarizes information about the
TOT conducted and provides basic profiles of the participants.
Master Trainers are strongly encouraged to send a copy to
the RPO. All TOTs that remain unreported, are not entered
into RPO’s data-bases and therefore not taken into account
when updating the Trainers database.
5. The
End of TOT Seminar Evaluation Form
Filled in by participants at the end of the TOT to provide
feedback to the Master Trainer about the level of appreciation
of the seminar.
6. The
